The designs you select from are defined by a Confluence global admin. If a design you need is missing or the app doesn't appear in the editor, contact your admin.
What you need

Confluence access: You need edit access on the page or blog post you are working on.

No admin rights required. Every task in this guide works without administrator permissions.

Your instance needs an active app license. Without one, a panel shows a license message instead of your content.
Which macro to use
The app provides two macros. They differ in where their design comes from, and that difference decides which one you want.
